HMO plans: confirm the PCP change
Update the primary care provider to Premier according to the plan rules and confirm when the change takes effect before the visit.
Confirm the effective date
Coordinate the first visit on or after the plan’s effective date.
Flag time-sensitive care needs
If your client needs medication or a specialist referral soon, call the team and flag the need in the form. This does not guarantee an earlier appointment.
Ask whether fasting is needed
Confirm clinic instructions for the tests being ordered. Patients should fast only if their healthcare professional instructs them to.
